Is there an actual rebate form that I will need to mail in with the receipt?
Note that it says "while supplies last". So if they decide to only honor the first 500 (or whatever amount) submissions and yours is received after that, you wont get a check and you'll be out whatever amount you paid. I am always leary of these types of rebates, especially if they dont come with an official form.
